About the model plan form
Your organisation can use this model conflict of interest and management plan form to develop a suitable form for its organisation.
Its main purpose is to provide a suitable form for employees to declare any conflict of interest they have (actual, potential or perceived), so that your organisation can develop and record a plan to manage the conflict in the public interest.
The form also sets out when an employee must complete the form to confirm that a conflict of interest does not exist.
This form is different to a ‘DOPI’ form – which requires a small percentage of employees (mainly executives and financial delegates) to routinely declare certain private interests, such as real estate.
