About the model plan form

This model plan form is for use by all employees.

Its main purpose is for you to declare any conflict of interest you have (actual, potential or perceived), so that your organisation can develop and record a plan to manage the conflict in the public interest.

This form is different to a ‘DOPI’ form – which requires a small percentage of employees (mainly executives and financial delegates) to routinely declare certain private interests, such as real estate.
